Helping Patients Live with Diabetes
The first step for successfully living with diabetes is understanding it. Patients who participate in decisions about their own health are better able to manage their diabetes on a daily basis.

At the Chronic Care Center, we strongly encourage patients to be a part of their own diabetes care team. To that end, we help empower patients by providing the education and support they need. We offer tailored educational programs to help patients learn as much as possible about diabetes and its treatment to better control their disease. We also offer support groups for children, adolescents and adults; giving them a chance to share their concerns about living with diabetes and help them cope with their stress.

A Team Approach to Care
Specialists at the Chronic Care Center work as a team to treat diabetes and its complications, such as heart disease, stroke, high blood pressure, blindness and kidney disease. We closely monitor our patients so that if complications arise, they will be detected early on and treated aggressively. All our specialists are experts in their own clinical areas and in the care of patients with diabetes. A patient who needs to see one of our specialists for diabetes related complications should rest assured that any recommended treatment will be decided with other members of his health care team. This helps ensure that recommendations complement rather than undermine the treatment of patients with multiple medical problems.
